Placebo-controlled trials are controversial when individuals might be denied existing beneficial medical interventions. In the case of malaria, most patients die in rural villages without healthcare facilities. An artesunate suppository that can be given by minimally skilled persons might be of value when patients suddenly become too ill for oral treatment but are several hours from a facility that can give injectable treatment for severe disease. In such situations, by default, no treatment is (or can be) given until the patient (...) reaches a facility, making the placebo control design clinically relevant; alternative bioequivalence designs at the facility would misrepresent reality and risk incorrect conclusions. We describe the ethical issues underpinning a placebo-controlled trial in severe malaria. To protect patients and minimise risk, all patients were referred immediately to hospital so that each had a higher chance of prompt treatment through participation. There was no difference between artesunate and placebo in patients who reached clinic rapidly; among those who could not, a single artesunate suppository significantly reduced death or permanent disability, a finding of direct and indirect benefit to patients in participating villages and elsewhere. Several countries maintain universal health coverage, which implies responsibility to organize delivery formats of healthcare services and products for citizens. In Brazil, the health system has a principle of universal access for more than 30 years, but many deficiencies remain and the country observes a day practice for those seeking judicial decisions to determine provision of healthcare.

In this paper we develop a typology of ‘non-commercial’ approaches to farming, based on a survey of a representative sample of farmers in Scotland, United Kingdom. In total, 395 farmers indicated that they do not seek to make a profit on their farms. We estimate that these non-commercial approaches to farming are utilised on at least 13% of agricultural land in Scotland. As such, non-commercial farming is not a marginal practice, nor are NCF limited to small-scale ‘hobby’ farms: NCF exist (...) across the scale of agricultural holding sizes and are operated by a wide range of socio-demographic cohorts. We identify 6 types of NCF: agricultural residences, specialist smallholdings, horsiculture holdings, mixed smallholdings, amenity mixed farms, and large farms or estates. These types were differentiated primarily by the scale of farm size, presence of diversification activities and types of animal present. The analysis demonstrates a number of emergent patterns of land management: de facto land abandonment, transition towards ‘horsiculture’, and management differences between retiring and new entrant NCF. We argue that the types identified reflect a number of intersecting issues in contemporary agrarian transitions, particularly the aging farmer population; generational renewal; and gendered implications of agricultural restructuring. Introduction: Although deep brain stimulation often improves levodopa-responsive gait symptoms, robust therapies for gait dysfunction from Parkinson's disease remain a major unmet need. Walking speed could represent a simple, integrated tool to assess DBS efficacy but is often not examined systematically or quantitatively during DBS programming. Here we investigate the reliability and functional significance of changes in gait by directional DBS in the subthalamic nucleus.Methods: Nineteen patients underwent unilateral subthalamic nucleus DBS surgery with an eight-contact directional lead in the most (...) severely affected hemisphere. They arrived off dopaminergic medications >12 h preoperatively and for device activation 1 month after surgery. We measured a comfortable walking speed using an instrumented walkway with DBS off and at each of 10 stimulation configurations at the midpoint of the therapeutic window. Repeated measures of ANOVA contrasted preoperative vs. maximum and minimum walking speeds across DBS configurations during device activation. Intraclass correlation coefficients examined walking speed reliability across the four trials within each DBS configuration. We also investigated whether changes in walking speed related to modification of step length vs. cadence with a one-sample t-test.Results: Mean comfortable walking speed improved significantly with DBS on vs. both DBS off and minimum speeds with DBS on. Pairwise comparisons showed no significant difference between DBS off and minimum comfortable walking speed with DBS on. Intraclass correlations were ≥0.949 within each condition. Changes in comfortable walk speed were conferred primarily by changes in step length.Conclusion: Acute assessment of walking speed is a reliable, clinically meaningful measure of gait function during DBS activation. Directional and circular unilateral subthalamic DBS in appropriate configurations elicit acute and clinically significant improvements in gait dysfunction related to PD. Next-generation directional DBS technologies have significant potential to enhance gait by individually tailoring stimulation parameters to optimize efficacy. (shrink)

A evolução dos meios de comunicação propiciou um massivo fluxo informacional, trazendo consigo alguns entraves. O fenômeno das fake news é produto dessa mudança na sociedade, afetando de diversas formas a vida das pessoas e inclusive a possibilidade de uma escolha plena e livre de influências, sua liberdade de expressão, pensamento e informação. Nesse contexto, este evento se torna gravoso para o nosso Estado Democrático, na medida que influencia o comportamento, as escolhas políticas, a conduta social e a solidez da (...) própria democracia. Diante do exposto, o objetivo do presente trabalho é refletir como as fake news interferem no Estado Democrático de Direito no Brasil e o deformam. Trata-se de uma revisão bibliográfica, documental e qualitativa. Propõe-se, por meio desse estudo, explicitar o que é esse fenômeno tão complexo das fake news e compreender as diversas formas de como ele pode afetar nosso sistema democrático, baseando-se nas ideias discursivas de Habermas. The power of the application of bioinformatics across multiple publicly available transcriptomic data sets was explored. Using 19 human and mouse circadian transcriptomic data sets, we found that NR1D1 and NR1D2 which encode heme‐responsive nuclear receptors are the most rhythmic transcripts across sleep conditions and tissues suggesting that they are at the core of circadian rhythm generation. Analyzes of human transcriptomic data show that a core set of transcripts related to processes including immune function, glucocorticoid signalling, and lipid metabolism is (...) rhythmically expressed independently of the sleep‐wake cycle. We also identify key transcripts associated with transcription and translation that are disrupted by sleep manipulations, and through network analysis identify putative mechanisms underlying the adverse health outcomes associated with sleep disruption, such as diabetes and cancer. Comparative bioinformatics applied to existing and future data sets will be a powerful tool for the identification of core circadian‐ and sleep‐dependent molecules. (shrink)

This paper investigates the implicative conditional, a connective intended to describe the logical behavior of an empirically defined class of natural language conditionals, also named implicative conditionals, which excludes concessive and some other conditionals. The implicative conditional strengthens the strict conditional with the possibility of the antecedent and of the contradictory of the consequent. $${p\Rightarrow q}$$ p ⇒ q is thus defined as $${\lnot } \Diamond {(p \wedge \lnot q) \wedge } \Diamond {p \wedge } \Diamond {\lnot q}$$ ¬ ◊ (...) ( p ∧ ¬ q ) ∧ ◊ p ∧ ◊ ¬ q. We explore the logical properties of this conditional in a reflexive normal Kripke semantics, provide an axiomatic system and prove it to be sound and complete for our semantics. The implicative conditional validates transitivity and contraposition, which we take to be integral parts of reasoning and communication. But it only validates restricted versions of strengthening the antecedent, right weakening, simplification, and rational monotonicity. Apparent counterexamples to some of these properties are explained as due to contextual factors. Finally, the implicative conditional avoids the paradoxes of material and strict implication, and validates some connexive principles such as Aristotle’s theses and weak Boethius’ thesis, as well as some highly entrenched principles of conditionals, such as conjunction of consequents, disjunction of antecedents, modus ponens, cautious monotonicity and cut. His career centred around Oxford – first as Tutor and Fellow at University College, then as Waynflete Professor of Metaphysical Philosophy at Magdalen College. His careful, thoughtful, and characteristically elegant written work was influential in moving Oxford philosophy from the anti-metaphysical leanings of A.J. Ayer and J.L. Austin to a renewed and rejuvenated era of traditional philosophy theorising, albeit domesticated in a distinctively Strawsonian fashion. His influence on (...) Objetiva-se no texto analisar o ensino de história na formação inicial e continuada, através do estágio docente obrigatório em espaços escolares, tematizando o racismo institucional e o sexismo. O conceito de racismo institucional será interseccionado com o de sexismo, pois a análise destaca fenômenos observados com jovens estudantes negras, licenciandas do Curso de História da Universidade Federal do Rio Grande do Sul. Sob a perspectiva da abordagem qualitativa, consideram-se as práticas pedagógicas, construídas em processos de ensino e pesquisa, desenvolvidas a (...) partir de autorreflexões e entrevistas abertas com estudantes negras em formação inicial. Problematiza-se a presença da branquitude, do sexismo e do racismo nas formas de viver e pensar essas práticas pedagógicas. Fundamenta-se em autoras como bell hooks, Grada Kilomba, Nilma Lino Gomes e Petronilha Beatriz Gonçalves e Silva, em associação com produções do campo específico do ensino de história no Brasil. As considerações parciais apontam a presença hegemônica das maneiras de agir e de pensar da branquitude no imaginário das comunidades escolares e universitárias, produzindo ações de discriminação racial e de gênero. In the section ‘Unity and Objectivity’ of The Bounds of Sense, P. F. Strawson argues for the thesis that unity of consciousness requires experience of an objective world. My aim in this essay is to evaluate this claim. In the first and second parts of the essay, I explicate Strawson's thesis, reconstruct his argument, and identify the point at which the argument fails. Strawson's discussion nevertheless raises an important question: are there ways in which we must think of our experiences (...) if we are to self-ascribe them? In the third part of the essay, I use Kant's remarks concerning the passivity of experience to suggest one answer to this question: in self-ascribing experiences, we must be capable of thinking of them as passive to their objects. Entendemos que nenhuma sociedade constrói o presente e alicerça o futuro sem compreender sua cultura e sem conhecer o significado de sua história. O objetivo deste artigo consiste em apresentar uma metodologia aplicada na sala de aula das primeiras séries do ensino fundamental, cujo propósito foi despertar a imaginação simbólica dos alunos através do uso das lendas presentes nos livros didáticos do ensino religioso ministradas pelos docentes. Os temas estudados têm como base teórica a fenomenologia da religião, a antropologia e (...) a teoria geral do imaginário. Selecionamos para nosso estudo como fundamentação metodológica a pesquisa descritiva, de campo e a abordagem qualitativa para análise dos dados. Ponderamos que a pesquisa descritiva observa, registra, analisa e correlaciona fatos ou fenômenos sem manipulá-los, ela procura descobrir, com precisão possível, a frequência com que um fenômeno ocorre, sua relação e conexão com outros, sua natureza e característica. Nos primeiros resultados de nossa investigação cientifica foi identificado que as lendas trabalhadas na sala de aula potencializam o imaginário popular resguardando a identidade cultural de uma manifestação religiosa. Consideramos que esta metodologia de ensino proposta pode ser implantada pelos professores do ensino religioso no seu planejamento. Palavravas-chave : Lendas. Imaginação. Educação.No society is able to build the present and set the bases for the future without understanding its culture and without knowing the meaning of its history. The purpose of this article is to present the methodology put into practice in the classrooms of the first grades of the elementary school, which have the sole intention to wake up the symbolic imagination of the students through the use of the legends found in the didactic books of the religious teaching ministered by the faculty members. The topics studied are based upon the religious phenomenology, anthropology and the general theory of imaginary. For our study, we used the field research and the qualitative approach as a methodological foundation, to make the data analysis. We consider that the descriptive research observes, registers, analyses and co-relates facts or phenomena without manipulating them; such approach helps us to discover the frequency in which a phenomenon happens, the connection of the phenomenon with the others, and also its nature and characteristics. Since the first results of our scientific investigation we have identified that the legends seen by the students in their classroom have potentized the popular imaginary and have maintained the cultural identity of a religious manifestation. O objetivo deste artigo é analisar a ética do cuidado de enfermagem no tratamento de pacientes oncológicos. Optou-se pela pesquisa bibliográfica de abordagem qualitativa cuja metodologia foi dividida em etapas. Na primeira etapa, foi realizado um levantamento de artigos publicados, no período de 2000 a 2009, em periódicos do Scielo, utilizando os descritores: saúde/doença, enfermagem, ética do cuidado e oncologia. Esse levantamento culminou com a seleção de 28 artigos, 21 dos quais foram eleitos posteriormente para leitura aprofundada, assim como para (...) utilização no estudo. Na segunda etapa, foram explorados dados do campo, com a transcrição e análise de depoimentos de profissionais da enfermagem sobre a concepção de cuidado bem como de dados sobre pacientes oncológicos, acompanhados no período de 2007 e 2008. A pesquisa e suas exigências protocolares, sob o número CAAE - 0147.0.213.000-10, foi aprovada pelo Comitê de Ética da PUC Minas em 31 de agosto de 2010. O referencial teórico foi composto por autores que discutem o cuidado na perspectiva filosófica e teológica - Boff (2001, 2005 e 2006), Efken (2005) - e autores da área da saúde oncológica, entre eles, Carvalho et al. (2002), Popim & Boemer (2005), Visentin, Labronici & Lenardt (2007) e Araújo et al. (2009). Os resultados evidenciaram o cuidado na enfermagem como princípio ético essencial, assim como a necessidade de investimentos em políticas públicas de saúde no Brasil, que efetivem o trabalho de prevenção, educação e cura do câncer. We have opted for bibliographical research of qualitative nature, a methodology divided into stages. Through this process the selection of twenty-eight articles was accomplished, however only twenty-one articles were utilized in the production of this study. In the second stage field data were explored, we transcribed and analyzed the statements of nursing professionals on the conception of care and on oncology patients overseen by them in the period of 2007 to 2008. The research protocol and its requirements under the number CAAE - 0147.0.213.000-10, was approved by the Ethics Committee of PUC Minas on August 31, 2010. The theoretical support was provided by authors who discuss care in a philosophical and theological perspective: Boff (2001, 2005, and 2006), Efken (2005); and authors in the field of healthcare, among them Popim & Boemer (2005), Silva et. al , (2005), Souza et al. (2005), Visentin, et al. (2007), and Araújo, et. al. (2008). The results demonstrated the care in nursing as an essential ethical principal, as much as the necessity of investment in public health policies in Brazil, for the effective promotion of cancer prevention, education and cure. Juventude e religiosidade: Cartografia dos processos de subjetivação de jovens católicos em uma comunidade de fé (Youth and religiousness: mapping the subjectivation processes of the catholic youth in a community of faith). Este estudo apresenta os resultados de uma pesquisa motivada pela constatação de que muitas subjetividades juvenis católicas do Brasil vivem uma situação de fragmentação que faz emergir distintos grupos identitários e em graves conflitos no seio das comunidades de fé. Por meio da realização de uma pesquisa-intervenção (...) numa paróquia católica na região metropolitana de Curitiba, procedeu-se uma cartografia dos processos de subjetivação (criação de modos de existência) da juventude católica nesta comunidade. Pretendeu-se com a cartografia, colocar em evidência tanto o modo como tais processos de subjetivação são forjados, quanto as linhas de fuga presentes nesses processos. A utilização da pesquisa-intervenção como tática para a realização da cartografia nos permitiu constatar que, mesmo no interior dos grupos identitários, há brechas no instituído por onde se pode fazer passar outras intensidades com vistas à promoção de modos de existência menos fechados e mais afirmadores de uma unidade que não abre mão da pluralidade e da diferença. Pesquisa-intervenção: The present study results from a research motivated by the fact that many Catholic youth in Brazil live a fragmented situation that brings out distinct identity groups and serious conflicts within communities of faith. By conducting a research intervention in a Catholic parish in the metropolitan region of Curitiba, we proceeded to mapping the subjectivation processes (creation of modes of existence) of Catholic youth in this community. It was intended to make a cartography to highlight both how such subjectivation processes are forged, as the lines of flight present in these processes. The utilization of intervention research as a tactics to perform the mapping allowed us to observe that even within the identity group there are gaps in the set where other intensities can go through in order to promote modes of existence less closed and more affirming the unit without losing plurality and difference. A pesquisa quantitativa tem sido predominante nos estudos sobre oração. A partir de uma abordagem qualitativa baseada na análise de conteúdo, este estudo examina as relações entre oração e saúde. Os dados são provenientes de 104 vídeos com entrevistas gravadas com participantes de igrejas Católicas, Protestantes e Pentecostais. A análise levantou quatro categorias descritivas do uso da oração: 1. Como estratégia de coping (enfrentamento) (62,5%); 2. Para manter viva a espiritualidade (15,3%); 3. Como técnica de empoderamento mútuo (8,6%); 4. Como (...) ponto de mudança no processo existencial (13,4%). Correspondem a estas categorias, respectivamente, os tipos de oração: 1. Petição, Lamentação; 2. Descanso, Sacramental, 3. Intercessão; 4. Conversão, Chamado; Movimento do Espírito. De acordo com a literatura corrente, a oração proporciona formas de conexão orientadas “para dentro/si mesmo” ; “para cima/divino” e “ para fora/outro” . Os resultados sugerem uma modalidade de conexão ainda não estudada: a conectividade epifânica – que se origina no sagrado, vem ao encontro do humano, e marca um ponto de mudança no processo existencial. Os achados indicam estreita relação entre oração e saúde mental e espiritual, como diminuição da ansiedade, direção e propósito na vida, e apontam a importância de maiores estudos na interface entre teologia e psicologia da religião. Using a qualitative approach based on content analysis, this study examines the relationship between prayer and health. Data come from 104 recorded interviews of participants from Catholic, Protestant and Pentecostal traditions. The analysis found four categories that describe the use of prayer: 1. Prayer as a religious coping strategy (62,5%); 2. Prayer as a discipline to keep spirituality alive (15,3%); 3. Prayer as a technique of mutual empowerment (8,6%); 4. Prayer as a turning point in the existential process (13,4%). The types of prayer corresponding to these categories are: 1. Petitionary and Lamentation; 2. Rest and Sacramental; 3. Intercessory prayer; 4. Conversion; Calling; Movement of the Spirit. According to the current literature, prayer provides inward, outward and upward connectivity. The results indicate an additional connectivity not yet studied: the Epiphanic connectivity, which comes from sacred to human and marks a turning point in the existential process. The outcomes suggest a close relationship between prayer and spiritual and mental health by decreasing anxiety, making meaning and purpose in life, and point out the relevance and need for further studies on the interface between theology and psychology of religion. What is the status of the claims which make up Kant’s arguments in the Critique of Pure Reason? This question seemed to Kant’s contemporaries to require a metacritique. Strawson’s criticisms of Kant should be understood in this context: as raising a metacritical challenge about Kant’s grounds for the claims which make up his arguments. What about the claims which make up Strawson’s own arguments in The Bounds of Sense? I argue in this chapter, against what I take to be the (...) general consensus, that Strawson did not and should not have understood these claims to be analytic. Rather he is somewhat puzzlingly committed to our possessing non-analytic but still a priori knowledge of his claims. What could such knowledge consist in? I’ll extract from G.E. Moore’s early writings on Kant a model for understanding such knowledge, one which enables us to better appreciate the way in which Strawson’s methodology dovetails with Kant’s own. (shrink)

Some comments on Quassim Cassam’s Self and World written for a conference at the Institute of Philosophy in 2017. I consider the objection that Cassam raises to Strawson’s argument from unity to objectivity in The Bounds of Sense and raise some general questions about Cassam’s problem of misconception and its application to transcendental arguments.

One of the abiding themes of the three essays which make up Iris Murdoch’s wonderful The Sovereignty of Good1 is that experience can be a way of our coming to possess aesthetic concepts. “We learn through attending to contexts, vocabulary develops through close attention to objects, and we can only understand others if we can to some extent share their [spatio-temporal and conceptual] contexts.” (IP, p.31). My interest in this paper is in what account of aesthetic experience can respect this (...) intuition; that “close attention to objects” can play an important role in our acquisition of aesthetic knowledge and concepts. I want to suggest that certain debates in the philosophy of mind can help us consider how aesthetic experience must be structured in order to play this role. (shrink)

Partimos do pressuposto que a arte é a revelação da profundidade do ser humano e que manifesta a sua busca pela verdade e pelo sentido de sua existência. O nosso objetivo foi descrever os fundamentos da arte barroca na consolidação e na propagação de verdades religiosas e políticas. Teremos como principal referência a igreja barroca de Santo Antônio, componente do Centro Cultural de São Francisco, localizada na cidade de João Pessoa, no nordeste do Brasil. A metodologia utilizada foi a pesquisa (...) descritiva tendo como instrumento a análise textual discursiva por meio da qual fizemos uma breve recapitulação do contexto nascente do barroco na Europa. Em seguida, apresentamos as suas características e, por último, as suas peculiaridades no Brasil. Como primeiro resultado da pesquisa foi possível considerar o barroco e o seu imaginário como fontes de verdade, superando a lógica binária do terceiro excluído, e, reconhecendo que o barroco funcionou como amálgama da sociedade colonial brasileira por meio das práticas religiosas e das vicissitudes políticas. This text aims to describe the basis of Baroque art in the consolidation and spread of political and religious truths. We will have as main reference the baroque church of St. Anthony, a component of the Cultural Center of San Francisco, located in the city of João Pessoa, in northeastern Brazil. The methodology used was descriptive and textual analysis as a tool through which we did a brief recapitulation of the context of the nascent Baroque in Europe. Then, we will present the characteristics of Baroque and finally its peculiarities in Brazil. As a first result of the research it was possible to consider the Baroque and its imagery as a source of truth, overcoming the binary logic of the excluded middle, and, recognizing that Baroque functioned as the amalgam of the Brazilian colonial society through religious practices and political setbacks. Child poly-victimization is a risk factor for psychopathology in adulthood, such as anxiety and depression. Despite that, there is minus investment regarding psychotic symptoms and the mechanisms explaining the relationship between poly-victimization and psychoticism. The purpose of this study is to investigate these variables and explain how they might be associated. A sample of 246 adults participated in this study, aged between 18 and 68 years (M = 37.5, SD = 12.5) and mostly females (76.8%). The results revealed a significant (...) mediating effect of anxiety and depression in the relationship between childhood victimization and psychoticism (b =.12, p =.011), but not a significant moderating effect of gender, Δχ2(5) = 3.87, p =.568. The affective pathway was supported by our findings. These results suggest the need of additional efforts on prevention of child victimization, as well as of intervention programs aiming to prevent its cumulative effect. (shrink)

We consider the simple case of a nonrelativistic charged harmonic oscillator in one dimension, to investigate how to take into account the radiation reaction and vacuum fluctuation forces within the Schrödinger equation. The effects of both zero-point and thermal classical electromagnetic vacuum fields, characteristic of stochastic electrodynamics, are separately considered. Our study confirms that the zero-point electromagnetic fluctuations are dynamically related to the momentum operator p=−i ℏ ∂/∂ x used in the Schrödinger equation.

In our paper we aim at reflecting upon the extent to which educational theory may be used as a framework in the analysis of policy documents. As policy texts are ‘heteroglossic in character’ (Lingard and Ozga, in The Routledge Falmer reader in education policy and politics, Routledge, London and New York, 2007 , p. 2) and create “circumstances in which the range of options available in deciding what to do are narrowed or changed” (Ball in, Education policy and social class: (...) The selected works of Stephen J Ball, Routledge, London and New York, 2006 , p. 46), they need to be theoretically tackled in their underlying assumptions and implications. This proposal draws on an analysis of two sets of documents of the European Union: texts produced between 2000 and 2006, underlying the European Union programmes; and texts produced by a working group focusing on the key competences of Lifelong Learning (2003–2006). Initially, the framework for the analysis of different documents was grounded on the existing research in the field of educational policy. Now we attempt a secondary analysis of the collected data by transposing the borders of this particular and highly prolific field. We argue that what is outside the texts may reshape what is inside the texts. Educational theory allows us to define some conceptual tools in order to question the documents as political apparatus which open and constrain possibilities. Therefore, we will use educational theory as an arena where different matters, perspectives and possibilities may be explored and assembled. We have engaged in a conversation with both the data and some theoretical approaches. Central to this conversation are the concepts of “ignorant schoolmaster” (Rancière, in The ignorant schoolmaster five lessons in intellectual emancipation, Stanford University Press, Stanford, 1991 ), “learning contexts” (Edwards, in Rethinking contexts for learning and teaching, Routledge, Oxon and New York, 2009a , b ), and “experience” (Larrosa, in Revista Brasileira de Educação, 19:20–28, 2002 ).
